Submit a Case
Send records through the Customer Portal, directly from your scanner platform, or by mail. Every case is reviewed before design or fabrication begins.
Before you send
Required records
Complete records at submission are the single biggest factor in a case moving without delay.
- Digital submission
- Upper and lower scans plus a bite registration, exported as STL or PLY, or sent directly from your scanner platform.
- Conventional submission
- Impressions or models with a bite registration, shipped with the completed prescription form.
- Prescription
- Appliance requested, arch, and every design option you want specified. Anything left open will be queried before fabrication.
- Construction bite
- Required for functional and sleep appliances, taken at the vertical and advancement you have determined.
- Return date
- The date you need the appliance in the practice, so production can be scheduled against it.
