Scan or impressions, a bite where the design needs one, and a signed prescription. If anything on the Rx is left open, a technician calls your practice before the case is started.

Print it or save it as a PDF, then send it with the case.

Download the Rx form or call the sleep lab, 623-533-3202.

The Original Snore Guard is made with a hard outer shell and a soft inner surface, so the patient gets the durability of a rigid appliance and the comfort of a soft one in the same piece.

The two shells are fused together in the posterior sections with the mandible held in a forward position.
